Surgery is typically indicated when:
- Leg pain persists beyond 6 to 12 weeks of conservative care and imaging confirms a disc herniation matching the symptoms
- There is progressive weakness (foot drop, difficulty rising on toes)
- Pain is intractable and disabling despite treatment
Emergency, not elective. New bowel or bladder dysfunction, saddle numbness, or rapidly progressive leg weakness can indicate cauda equina syndrome. That is an emergency requiring decompression within hours, not a procedure to price-shop or travel for.
What drives the price
- Fusion hardware. Pedicle screws, rods, and interbody cages are the single biggest variable in spine pricing. In US hospitals they are marked up heavily; transparent centers sell them at cost; Colombian bundled quotes usually itemize hardware separately.
- Facility type. Microdiscectomy is routinely done outpatient at an ambulatory surgery center at 30 to 50 percent below hospital pricing. Fusion and Chiari decompression require a hospital.
- Levels. Each additional level adds roughly 40 to 70 percent to fusion cost.
- Neuromonitoring. Intra-operative nerve monitoring adds $1,000 to $3,000 in the US and is usually bundled in Colombia.
- Surgeon specialty. Both neurosurgeons and orthopedic spine surgeons perform these; fees are comparable. What matters is fellowship training and case volume in the specific procedure.
Traveling for spine surgery: an honest note
Microdiscectomy and single-level decompression travel well: outpatient or one night, walking the same day, cleared to fly within 7 to 14 days, and a low revision rate. Fusion is different.
